The individuals in our units can be;
- Step-Down young adults who were previously looked after by Local Authorities. We are able to provide the relevant support/life skills training for their transition into adulthood so they can live independently.
- Emergency accommodation- i.e following breakdown of previous supported lodgings. This may also include trafficked individuals and those who are fleeing domestic abuse and exploitation.
- Adults who are unable to function independently and require additional support and empowerment to live independently.
- Adults who have been released from prison and need support and integration back into the community
- Adults who have been discharged from hospital
- Adults who have behavioural problems, low level concerns around their mental health, substance misuse, self-harm and violent or disruptive behaviours.
- Asylum seekers
SSY offer a range of services for this service user group which include services/support delivered by in-house staff and outside agencies (specialist support). We understand, empower, engage and support individuals by working with them to embed the necessary life skills to live independently. The work completed by SSY is regularly reviewed and measured by using methods such as regular staff supervisions, in-house audits completed by management and 6 monthly/ yearly independent inspections as this will ensure that individuals needs are being met and ensures that service users are getting the relevant support as per their plan.
SSY will keep daily/weekly logs for each individual and also provide monthly updates to relevant professionals if needed i.e. social worker, probation worker etc.). SSY will liaise with linked professionals, attend any scheduled meetings/reviews and relay concerns that may arise for the individual to all key parties and adjust any plans as per service user needs.
Support we provide includes and is not limited to;
- Shopping
- Budgeting and managing finances
- Cooking
- Domestic skills (cleaning, ironing, changing sheets etc.)
- Planning and diary keeping to ensure relevant appointments are attended
- Accompany and assist with transportation to appointments
- Support in registering with local services
- Assist with maintaining or seeking further education/apprenticeship/training and employment.
- Assist with applying for benefits
- Referring to partner agencies if needed which include, drugs and alcohol support, counselling, MH services and domestic abuse support
- Building confidence and self-esteem
- Gaining and maintaining tenancy
This service is aimed at vulnerable adults in either female/male only homes or mixed units and is designed for individuals who are unable to live independently and require the support that will assist them in acquiring the relevant skills and confidence in living independently. We assist people from all walks of life regardless of their gender, ethnicity, cultural and religious background. Our package of services will be bespoke to their individual needs taking into consideration their risk assessment and initial assessment as well as liaising with key-professionals. We provide short and long-term placements based on individual needs.
